About The Project
Contact Health has over 20 years of experience in providing supportive healthcare and health monitoring solutions.
They wanted a dedicated, web-based Remote Patient Monitoring and Care Management System that would provide a safety net for at-risk individuals living at home.
Goal
The client’s goal was a system that could assess the journey of the patient from one point in time to another. In this way, the system had to create a holistic timeline with all the vital statistics, such as blood pressure, pulse, weight, glucose, and temperature, over an observable period. The system would then map these values against a normal-value chart. If the patient data showed a variation from normal, the system would alert the remote care staff, who could then take remedial action to address the patient’s health needs.


How It Works
Then Remote Patient Monitoring and Care Management System sends parameters to medical service providers taken by the Patient from their Device (TAB/ Mobile ) which are Reflect in Healthcare Dashboard . Upon the receipt of patients’ data, medical service providers perform their analysis and suggest an appropriate care plan.
Care plans include medicinal prescriptions, a periodic questionnaire containing health queries, and other engaging resources for the patient.
Let's Understand All The Platforms

This Web Application is basically for the Service provider Of Contact Health. When A patient take vital records from their devices that records will display on the dashboard. If the patient data showed a variation from normal, the system would alert the remote care staff, who could then take remedial action to address the patient’s health needs.
2.Monitoring Device For Patients (TAB):
Patients are provided with an Android Tablet to record vitals, complete surveys, receive medication reminders, and communicate with the Care Team. To record vitals, patients simply need to connect their devices (Weight Scale, Blood Pressure Cuff, Pulse Oximeter, Glucose Meters, Spirometer) to the tablet via Bluetooth and input their assigned readings for the day. These readings will then be visible on the Service Provider dashboard.
3.Monitoring Device For Patients (Mobile):
Patients can monitor their vital signs using their own phone by installing the application from the Play Store or App Store, same by connecting the bluetooth .
4.Monitoring Device For Caregiver (Mobile):
An application has also been developed for family members (caregivers) to track or update the health status of their loved ones. This can be done by answering surveys, checking medication reminders, and monitoring upcoming and missed medications and vitals.

The Old dashboard design was outdated, and the client is looking to revamp it with more detailed information to eliminate the need for excessive scrolling by users.

Here, a Care team member can view the patients' details to identify any issues in their readings and take appropriate action based on the information.

Edit Health Programme:
At this point, a Care Team member is responsible for assigning tasks to patients, such as measuring vital signs and responding to survey questions, based on their results. This also involves determining the frequency for displaying the questionnaire and when to initiate it.
Add New User to Care Team:
On this platform, users have the capability to establish a personalised profile tailored to their specific needs and preferences. By initiating the creation of a new user profile, individuals can input a comprehensive array of information, encompassing personal details, contact information. Moreover, users can define and allocate access roles within their profile, granting specific permissions and privileges based on their requirements and responsibilities. This process ensures that each user's profile is meticulously customised to reflect their unique identity, facilitating a seamless and efficient user experience within the system.

Referral of patient & Send Consent In Dashboard:
This dashboard displays a list of referred patients, allowing you to view their information and send them consent via phone message, email, or other methods, along with the necessary devices based on their health condition.
You can track the status of these patients by clicking on the activity tab.
